چکیده

A 17 years unmarried female presented to Gastroenterology Department of our hospital for nonspecific symptoms of epigastric pain. The Ultrasonographic examination of the patient revealed triple kidney with horseshoe anomaly. She was then referred to us for further evaluation. We further proceed with Intravenous Urography (IVU) which demonstrated a bilateral duplex system in a asymmetrical horseshoe kidney (L shaped fusion). Left side kidney was vertically placed while the right side kidney was horizontally placed. One of the moieties was on the right side of vertebral column while the other moiety was overlying the vertebra and isthmus was on the left side of vertebra (Figure-1). A contrast enhanced computed tomography (CECT) scan confirmed the diagnosis of bilateral duplex system in horseshoe kidney (Figure-2). As the patient had non-specific, unrelated symptoms, patient was give symptomatic treatment for the same and was advised regular follow-up. Duplicated collecting systems (also known as duplex collecting systems) can be defined as renal units containing two pyelocaliceal systems that are associated with a single ureter or with double ureters. The prevalence of Partial duplication is 0.6%, while complete duplication of ureters occurs in 0.2% of live births (1). When a single ureteral bud bifurcates before the ampulla bifurcates, a duplex kidney with a bifid renal pelvis or bifid ureter results (2). Congenital anomalies of the kidneys include a group of so-called fusion anomalies, in which both kidneys are fused together in early embryonic life. Fusion anomalies of the kidneys can generally be placed into 2 categories: [1] horseshoe kidney and its variants and [2] crossed fused ectopia. Horseshoe kidney is probably the most common fusion anomaly. The term horseshoe kidney refers to the appearance of the fused kidney, which results from fusion at one pole. Horseshoe kidney is the most common renal fusion anomaly, the incidence being about 1

Xanthogranulomatous pyelonephritis in a horse-shoe kidney.

Xanthogranulomatous pyelonephritis (XGPN) represents an unusual suppurative granulomatous reaction to chronic infection, often in the presence of chronic obstruction from a calculus. We present a case of XGPN in a horse shoe kidney in an adult. Hemi-nephrectomy of the involved side was followed by clinical improvement. The case highlights the importance of early hemi-nephrectomy in XPGN with ho...

Prenatal Diagnosis of Bilateral Fetal Duplex Kidneys and a Short Review about Duplex Kidneys

Duplex kidney is one of common major congenital abnormalities of the urinary tract with an incidence about 1% of all live births and higher frequency in females than in males. Majority of duplex kidneys are unilateral. Although the antenatal diagnosis of fetal hydronephrosis is simple, the diagnosis of fetal renal duplication is infrequent, especially in case the collecting system is not dilate...

Neurofibromatosis-I Presentating with Multiple Spinal and Intracranial Neurofibromas.

Von Recklinghausen's neurofibromatosis (NF-1) is a phacomatosis characterised by widespread nervous system tumours with cutaneous manifestations and variably associated anomalies. We report here a case, who, in addition to classical features of NF-1 (café-au-lait spots, cutaneous and subcutaneous neurofibromas) demonstrated radiological evidence of both spinal and intracranial neurofibromas and...

Percutaneous nephrolithotomy (PCNL) in horse shoe kidneys.

OBJECTIVE To review our experience of PCNL in horse shoe kidneys. METHODS Between June 2001 and January 2005 we performed PCNL in 16 Horse shoe kidneys in 14 patients with calculi, 2 patients had bilateral calculi. Percutaneous puncture was made with patient in prone position. Their ages ranged between 4 to 52 years with a mean age of 26.3 years. Our mean stone burden was 820 mm2 (range 40-13...

Pancake kidney: An unusual type of renal fusion anomaly found in 99mTc-DMSA renal scintigraphy

A 2.5-year-old boy with a history of the previous incidental finding of an ectopic kidney in ultrasonography was referred to our nuclear medicine center for 99mTc-DMSA renal scintigraphy. The scan showed an unusual type of ectopic fused kidneys on the right side of the lower abdomen. In this anomaly, kidneys were fused completely across the medial portion of both upper and lower pole...
